Care managers assist patients in navigating the complex health care system, and coordinate care across the continuum with team members. There is an integrative team-based approach to your health care for the best possible outcomes with the highest quality care. The Care Management department responds to the medical management and psychosocial needs of all patients served.

Our team provides care planning services, to ensure that after you leave the hospital, your care and complex conditions are managed. We ensure that our patients and families are supported through the health care system by involving them in care planning decisions. As a team, we take patient rights seriously.

We are committed to optimizing community resources for you and your family and helping you navigate the health care system. Our referrals to outside agencies include homecare, community services, and assistance with ancillary services. We focus on prevention so that you can manage your illness and prevent re-hospitalization of any acute illness.

Patient Responsibilities and Rights

Patients have the right to receive information about their health care and must provide consent before medical information can be shared. While care managers are dedicated to assisting patients, patients are ultimately responsible for their own care and may refuse services when necessary.
